These SMS Terms and Conditions govern participation in the Livesworth Healthcare text-message program known as Livesworth Referral Alerts. By requesting or consenting to receive text messages from Livesworth Healthcare (“Livesworth,” “we,” “us,” or “our”), you agree to these terms.
1. Program Description
Livesworth Referral Alerts is a low-volume operational text-message program used to notify Livesworth’s authorized administrator when a referral-readiness inquiry is submitted through the Livesworth website.
Messages may identify Livesworth Healthcare, provide a generated reference number, and direct the authorized administrator to a secure intake worklist. The program is not intended for advertising or third-party marketing.
2. Consent and Enrollment
The recipient directly requests these operational alerts and provides the mobile telephone number during Livesworth’s administrative system setup. Consent is documented in Livesworth’s administrative records.
Website visitors, patients, children, parents, guardians, referral sources, and other individuals are not automatically enrolled in this SMS program merely because they use the Livesworth website or submit an inquiry.
Consent to receive text messages is voluntary and is not a condition of purchasing or receiving services from Livesworth.
3. Types of Messages
Messages sent through Livesworth Referral Alerts may include:
- Notification that a referral-readiness inquiry has been received.
- A generated reference number associated with the inquiry.
- Instructions directing an authorized administrator to a secure intake worklist.
- Administrative confirmations or system-related notifications concerning the alert program.
SMS alerts are not intended to contain patient names, child information, diagnoses, treatment information, clinical details, medical records, or other protected or sensitive health information.
4. Message Frequency
Message frequency varies depending on the number of referral-readiness inquiries and related operational events. Because messages are event-triggered, no fixed number of messages is guaranteed.
5. Message and Data Charges
Message and data rates may apply. Your mobile carrier’s standard messaging and data charges may apply to messages sent or received through this program. Livesworth is not responsible for charges imposed by your mobile carrier.
6. Opting Out
You may stop receiving messages at any time by replying STOP to a Livesworth Referral Alerts message.
After submitting a STOP request, you may receive a final confirmation message stating that you have been unsubscribed. No additional messages will be sent unless you later provide new consent to receive them.
Other supported opt-out words may include CANCEL, END, QUIT, REVOKE, OPTOUT, or UNSUBSCRIBE.

8. Supported Carriers and Delivery
Text-message delivery is subject to mobile-network availability and carrier support. Wireless carriers are not responsible for delayed or undelivered messages. Livesworth cannot guarantee that every message will be delivered or received within a particular timeframe.
9. Mobile Information and Privacy
We do not sell, rent, share, or provide mobile telephone numbers, SMS opt-in information, or messaging consent records to third parties or affiliates for their marketing or promotional purposes.
We may provide limited mobile information to telecommunications, messaging, hosting, security, or technical service providers solely as necessary to transmit, administer, secure, and support the Livesworth Referral Alerts program. We may also disclose information when required by law.
Text-message originator opt-in data and consent will not be shared with unrelated third parties.

10. Security and Sensitive Information
Text messaging is not guaranteed to be a secure communication method. Recipients should not reply to an alert with patient names, child information, diagnoses, medical information, clinical details, Social Security numbers, financial information, or other sensitive information.
Authorized personnel should access referral information only through Livesworth’s designated secure intake system.
11. Program Changes or Termination
Livesworth may modify, suspend, or discontinue the Livesworth Referral Alerts program at any time. We may also update these SMS Terms and Conditions to reflect operational, legal, regulatory, carrier, or technology changes.
Updated terms will be posted on this page with a revised “Last Updated” date. Continued participation after an update constitutes acceptance of the revised terms, subject to applicable law.
