The Livesworth referral-readiness form stores limited information so intake personnel can review an inquiry and call the referring person. Detailed medical records and orders are exchanged separately through an approved secure method.
AT A GLANCE
This notice applies to information submitted through this website. It does not replace privacy notices that may apply after a patient is accepted for services.
Referral-source contact details, a child reference or initials, age range, timing, requested service, selected care needs, schedule, and general readiness notes.
To review the inquiry, call the referring person, identify missing information, consider potential service fit, and arrange the next step.
Access is intended for authorized Livesworth intake personnel through an authenticated intake worklist and for service providers supporting the website.
Full patient names, medical records, orders, medication lists, insurance identifiers, and document uploads should not be submitted through this form.

When a referral-readiness inquiry is submitted, Livesworth stores the information entered in the form along with a referral reference number and submission date and time. This may include:
A child’s full name, exact date of birth, Social Security number, insurance member or policy number, detailed diagnosis or medical history, medication list, medical records, orders, or documents.
Livesworth uses website referral information to acknowledge and organize the inquiry, contact the referring person, identify missing information, conduct a preliminary service and readiness review, coordinate follow-up, maintain a referral worklist, protect the website, and meet applicable legal or operational obligations. A submission does not establish a patient relationship or confirm acceptance, authorization, staffing, or a start date.
Submitted referral information is retained in the database supporting this website. The intake worklist requires authenticated access and is intended for authorized Livesworth personnel. Technology providers that support website hosting, authentication, database operations, security, notifications, or maintenance may process information on Livesworth’s behalf as necessary to provide those services.
When notifications are enabled, the email and text alerts contain only the referral reference number and a direction to review the authorized worklist. They do not include the child reference, contact information, care needs, schedule, or medical details.
No website or internet transmission can be guaranteed completely secure. This preliminary form is not represented as a full clinical intake system or a HIPAA-secure medical-record upload portal. Livesworth will arrange a separate approved method when medical records, orders, or other detailed health information are needed.
When you apply for a role, we collect the contact details, role, reported experience, skills, service area, and availability you review and submit, together with your CV if attached, your confirmation, a reference number, and submission time. CV text is read in your browser to help fill the application. You can correct the details before submitting or apply without a CV.
Submitted applications and attached CVs are stored for authorized Livesworth hiring personnel to review, contact you about opportunities, and manage recruitment. CV downloads require authorized access. Our hosting and storage providers process this information to operate the service. After submission, our email provider processes your email address and application reference to send a confirmation; the hiring team receives a reference-only alert. CVs are not attached to these emails. We do not use a paid AI service to read these CVs. Do not submit patient information, Social Security numbers, or identity documents in your application. Applying does not guarantee employment or available shifts.
We keep applications while needed for recruitment and related operational obligations. For questions, corrections, or deletion requests, call Livesworth at (346) 341-8231. You may also call instead of using the application form.
Referral information may be retained for as long as reasonably necessary to evaluate and document the inquiry, complete follow-up, maintain security, and satisfy applicable legal, regulatory, or operational requirements. To ask about information submitted through the website, request a correction, or request deletion when permitted, call (346) 341-8231.
You do not have to use the online referral form. You may call Livesworth during normal business hours to begin a referral conversation. Please consider whether you are authorized to provide the information before submitting it.
